September 27-29, 2002- The 3 DAY conference fee includes both workshops, 3 continental breakfasts, 2 lunches, and 6 breaks, PLUS $242 worth of mentoring & induction publications, the mentoring program materials display, and a free 30 minute private consultant appointment. (Only 4 appointments are available to the first 4 programs who pay their registration fee and request an appointment.)

A. AFTER you press the submit button (in step 3 below), you will need to WAIT a few seconds.

Then you'll see a summary of your registration info.

You can print out that form to save a copy of your choices and costs

OR B. print out this current page BEFORE you click on "Submit".

3. When ready to send your initial registration, click on the "SUBMIT" button below.

Clicking on "Submit" sends us an email with all your registration information.

We will hold your space for ONE WEEK to give you time to send us your check OR a Purchase Order.

We can not guarantee your space if your check or P.O. arrives later than 1 week after your tentative email registration form.

hold your space until the original signed P.O. arrives in the regular mail.

4. To FINALIZE your registration, send a check OR purchase order

MADE OUT TO "BEST PRACTICE RESOURCES".

Only when we receive that check or faxed PO will your space actually be reserved for the conference.

At that time a written confirmation amd other details will be mailed to you.

YOU MAY USE ONLY ONE KIND OF DISCOUNT, either a TEAM or an EARLY DISCOUNT.

TEAM DISCOUNT

Team registration saves each person on the team $10 off the registration fee. You will receive this discount IF...

1. TWO OR MORE persons from the same organization register for the same conference.

2. Each person on the team completes and submits their own "Registration Form" with the same organization name.

AND...

3. ONE Check or ONE P.O. for conference registration fees is sent to pay for the whole team at the same time and lists the team members' names.

NOTE:

• The check for the conference registration fees should be received within a week of sending the tentative electronic registration forms.
• The space for the team is only guaranteed when payment or a faxed PO is received.
• A faxed P.O. followed by actual P.O. meets this requirement. FAX to 630-752-9941.

EARLY REGISTRATION DISCOUNT

YOU MAY USE ONLY ONE KIND OF DISCOUNT, either a TEAM or an EARLY DISCOUNT.

 

To receive the "Early Registration Discount" the CHECK OR PURCHASE ORDER for the

conference registration must be post marked BY AUGUST 16, 2002. Faxed P.O.s are OK but must be received by Aug. 16, 2002

 

Individuals for whom these requirements are met may deduct the "Early Registration Discount" of $10 per person.

Cancellation policy - We understand that unforeseen events may change a person's ability to attend the conference for which they have registered. Therefore, we will refund registration fees according to the following policy and to the extent that we have not already incurred costs for the registrant.

For example, after a certain date we must guarantee numbers to the hotel and pay for that number whether that many people actually come and eat anything. This cost totals mega dollars! Also, materials and supplies must be purchased and conference handouts must be copied several weeks in advance to allow the time needed to work with the printer and to build all the conference binders. Another concern occurs when we have reached the maximum registration and have begun to turn potential participants away. It is unfair if we then lose your business at the last moment and we do not have time to replace your registration. There are other issues in addition to these. That is why a cancellation policy is needed.

ALL CANCELLATIONS MUST BE MADE IN WRITING. EMAIL IS OK.

Refunds are calculated as follows:

• Written cancellation is received by August 16, 2002 = refund the registration fee less $20.
• Written cancellation is received by August 29, 2002 = refund the registration fee less $50.
• Written cancellation is received by September 16, 2002 = refund the registration fee less $100.
• Written cancellation is received by September 19, 2002 = refund the registration fee less $125.
• NO registration fees will be refunded if cancellations are received after September 24, 2002.

NOTE: The rate for the block of overnight guest rooms is only guaranteed until September 12th, 2002 so

Note: Hotel guests receive a complimentary continental breakfast in the hotel lobby.

Conference participants who do NOT stay at the hotel receive their continental brealfast in the training room.

Transportation to the conference site

The Harrisburg PA airport serves the greater Harrisburg area and our conference site.

• Driving in and need a map to the conference site? Go to mapquest.com and enter the address and zip code for the hotel above.
• Flying in and need a SHUTTLE from the airport to the hotel? - FREE shuttle transportation via van between Harrisburg Airport tand the conference site is provided by the Comfort Inn- Hershey, but you should CALL 717-566-2050 & SCHEDULE THAT SHUTTLE IN ADVANCE to be sure they can accomodate your schedule. This is especially true at the close of the conference.
