April 18-21, 2001- The FOUR DAY conference fee includes all three workshops, 4 continental breakfasts, 4 lunches, and 8 breaks, $163 worth of mentoring & induction publications, the mentoring program materials display, and a free 30 minute private consultant appointment. (Only 9 appointments are available to the first 9 programs who pay their registration fee and request an appointment.)

 

Dinner? For an additional cost of $25 each you may join your colleagues for a fine dinner and informal program on any of the evenings of April 18-20.

Complete the registration form, select one form of payment, and click on "Submit" to send your registration.

NEXT -

1. Be sure that you have completed all of the Registration Form provided above.

 

2. Make a copy for YOU: AFTER you press the submit button (in step 3 below), you will need to WAIT a few seconds.

Then you'll see a summary of your registration info.

You can print out that form to save a copy of your choices and costs OR

print out this current page BEFORE you click on "Submit".

 

 

3. When ready to send your tentative registration, click on the "SUBMIT" button below.

Clicking on "Submit" sends us an email with all your registration information.

We will hold your space for ONE WEEK to give you time to send us your check OR a Purchase Order.

We can not guarantee your space if your check or P.O. arrives later than 1 week after your tentative email registration form.

 

4. To FINALIZE your registration, send a check OR purchase order

MADE OUT TO "BEST PRACTICE RESOURCES".

 

Mail it to Best Practice Resources, 26W413 Grand Ave. Wheaton, IL 60187.

 

A FAXED P.O. with a P.O. # and signature will hold your space until the original P.O. arrives in the regular mail.

 

Only when we receive that check or faxed PO will your space actually be reserved for the conference.

At that time a written confirmation will be mailed to you.

TEAM DISCOUNT

Team registration saves each person on the team $10 off the registration fee. You will receive this discount IF...

1. TWO OR MORE persons from the same organization register for the same conference.

2. Each person on the team completes and submits their own "Registration Form" with the same organization name.

AND...

3. ONE Check or ONE P.O. for conference registration fees is sent to pay for the whole team at the same time and lists the team members' names.

NOTE:

• The check for the conference registration fees should be received within a week of sending the tentative electronic registration forms.
• The space for the team is only guaranteed when payment or a faxed PO is received.

EARLY REGISTRATION DISCOUNT

YOU MAY USE ONLY ONE KIND OF DISCOUNT, either a TEAM or an EARLY DISCOUNT.

 

To receive the "Early Registration Discount" the CHECK OR PURCHASE ORDER for the

conference registration must be post marked BY MARCH 14, 2001

 

Individuals for whom these requirements are met may deduct the "Early Registration Discount" of $10 per person.

Cancellation policy - We understand that unforeseen events may eliminate a person's ability to attend the conference for which they have registered. Therefore, we will refund registration fees according to the following policy and to the extent that we have not already incurred costs for the registrant.

For example, after a certain date we must guarantee numbers to the hotel and pay for that number whether that many people actually come and eat anything. This cost totals thousands of dollars! Also, materials and supplies must be purchased and conference handouts must be copied several weeks in advance to allow the time needed to work with the printer and to build all the conference binders. Another concern occurs when we have reached the maximum registration and have begun to turn potential participants away. It is unfair if we then lose your business at the last moment and we can not replace your registration. There are other issues in addition to these. That is why a cancellation policy is needed.

ALL CANCELLATIONS MUST BE MADE IN WRITING. EMAIL IS OK.

Refunds are calculated as follows:

• Written cancellation is received by March 1, 2000 = refund the registration fee less $20.
• Written cancellation is received by March 14, 2001 = refund the registration fee less $50.
• Written cancellation is received by April 1, 2001 = refund the registration fee less $100.
• Written cancellation is received by April 10, 2001 = refund the registration fee less $125.
• NO registration fees will be refunded if cancellations are received after April 14, 2001.

Transportation from the airport to the conference site

Details about this will be sent to you in your registration confirmation packet.

O'Hare and Midway are the two major airports serving the greater Chicago area.

• O'Hare Airport is about 20-30 minutes from the conference site and is served by a long list of the major and minor air carriers.
• Midway Airport is a smaller airport, is served by American, Southwest, and most of the least expensive airlines, such as ATA. It is about 45 minutes away from the conference site.

SHUTTLES?

The least expensive means of transportation to the conference site is provided by AIRPORT EXPRESS which runs shuttle vans to and from both O'Hare and Midway Airport.
